Proof of address

Naomi is offering individuals who require an affidavit to prove their address and identity.

This is an option as per Elections Canada when applying for Mail-In Voting Ballots. You can book a time here.

Elections Canada must receive your application either online or via mail by Tuesday September 14 @ 6:00 PM.

Naomi can help prepare your affidavit in a format to file online.

Naomi is only offering these affidavits remotely, until September 10, 2021 and only at certain times on a first come, first served basis.

You must have access:

  • To high speed internet

  • To a computer and

  • To connect to a video conferencing platform/app (zoom/MS Teams).

Naomi, unfortunately, cannot help if you do not have access to the above.

On-Reserve First Nations individuals should contact their Band.

Naomi will retain your affidavit in accordance with her privacy policies and rules governing lawyers. Naomi will not use your information for any other purpose other than to coordinate signing. Naomi is not associated with Elections Canada.

Do not book a time if you cannot keep it.

Clarification [2021.08.28]: No cost to complete. There is, however, a small cost if you book, cancel then try to rebook - this is why you should not book unless you are certain.
